Who can serve as a witness?

For the PAR-Q+ (Physical Activity Readiness Questionnaire for Everyone), a witness on our pre-participation screening form may vary depending on the organization or jurisdiction using it. Generally, the following individuals can serve as a witness:

  1. Licensed Healthcare Professionals – A physician, nurse, physiotherapist, or other regulated health professional may act as a witness.
  2. Qualified Exercise or Fitness Professionals – A personal trainer, kinesiologist, or exercise physiologist (e.g., HFFC, ACSM, NSCA) may act as a witness.
  3. Employer or Facility Representative – If used in a gym, workplace, or recreational setting, a manager, HR representative, or facility staff member may act as a witness.
  4. Family Member or Trusted Individual – A non-medical individual, such as a family member or trust individual, may witness the signature, depending on the policies of the organization requiring the form.

It is important to verify the specific PAR-Q+ usage guidelines of the organization requesting the form, as some may require medical clearance via the ePARmed-X+, a qualified exercise professional, and/or a healthcare professional.
